This process is essential in ensuring that a workforce is protected from harm and that an organisation is meeting the legal requirements set out by health and safety law.

This online learning course provides an introduction to incident investigation and shows how non-complex incidents can be investigated effectively. It is suitable for managers and supervisors who are responsible for implementing and monitoring risk control measures at work.

Accident Investigation Training - Course Overview


Statistics from the HSE (Health and Safety Executive) show that 693,000 people sustained an injury at work during 2019/20. Accident investigation is an important tool that employers can use to understand any events that did, or had the potential to, cause injury, and use this information to prevent future accidents, occupational diseases and property damage to work premises.

In this accident investigation course, we will:

  • Define the key terms associated with accident investigation, including adverse events and near misses.
  • Give the reasons for investigating accidents and incidents.
  • Discuss the benefits of investigating adverse events.
  • Define the terms ‘immediate,’ ‘underlying’ and ‘root’ causes.
  • Cover the actions to be taken following an adverse event before an investigation can take place, such as incident reporting.
  • Discuss the stages of a structured approach to adverse event investigation.


It takes approximately 1 hour and 30 minutes to complete the course. This can be spread out over multiple sessions or completed in a single session, and there is no time limit for completion.


All candidates must complete a multiple choice assessment after working through the training material. The candidate will be asked 12 multiple choice questions and need to answer a minimum of 9 correctly to pass. Candidates will be given instant feedback after completing the assessment stating whether they have passed or failed. The assessment can be retaken as many times as is required to pass at no additional cost.

  • What adverse events are.
  • What causes adverse events to occur.
  • Why adverse events should be investigated.
  • The structure of a typical investigation.

This module explores:

  • What to do before beginning an investigation.
  • How to report accidents under RIDDOR (the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 2013).
  • The information that should be gathered as part of an investigation.
  • How to ensure that the information gathering process is completed thoroughly.

This module explores:

  • How to use the ‘why’ method to analyse information.
  • How to establish the immediate, root and underlying causes of an adverse event.
  • How to evaluate potential risk control measures.
  • What action plans are, and how they are created as part of an investigation.

This module works through an example of an adverse event investigation to explore how they work in practice.

Accident Investigation Training - Accreditation and Certification

IIRSM Certificate

Successful candidates will receive a IIRSM Certificate and 2 CPD Credits.

This course certificate has no expiry date because there is no formally recognised period of validity for any health and safety training or this type of course. The HSE recommends refresher training is carried out whenever it is felt necessary. Safety registration bodies such as CHAS, SafeContractor, ConstructionLine & SMAS and other members of the SSiP scheme recommend that most re-training is done every 3 years. We would recommend refreshing knowledge as and when legislation or guidance changes applicable to these regulations.
